Vaden Health Center located at 866 Campus Drive, is a state-of-the-art facility that features the latest technology and a staff of professionals to provide you with excellent health care. The center offers medical services, counseling and psychological services, health promotion services, and health insurance, to foster the health and well-being of Stanford students. Six Must-Do Requirements Before Entering Stanford

Before you can become a Stanford student, there are six online medical forms you must complete to initiate your electronic health record. 

Information and login instructions about the six entrance medical requirements can be found at the Vaden Health Center web site.

Deadline: The deadline for completing your six entrance medical requirements is July 31. International students have until September 25 to complete and submit tuberculosis screening documentation only; all other requirements must be completed by July 31.

If you fail to meet the deadline, a hold will be placed on your future enrollment; this means that you will not be able to enroll for classes, and you will incur additional registration fees, lose pre-selected classes, and your loan and stipend checks will be interrupted.
